If you elect dental coverage you may switch between the DMO and PPO as often as monthly.

Dental Maintenance Organization (DMO)

This choice offers unlimited benefits, with no deductible, including 100 percent coverage for preventative and basic services and 60 percent coverage for restorative services. Orthodontia benefits are available for covered children with no lifetime maximum. You must choose a Primary Care Dentist who is responsible for performing your basic dental care and managing all your dental care services, including referring you to specialists when necessary.

Dental PPO

This choice offers benefits limited to $1,500/year with a $50 annual deductible ($100 per family) and coinsurance (the plan pays 50 to 80 percent of the cost of most services). Orthodontia benefits are available for covered children up to a lifetime maximum amount of $1,500 (separate from the annual maximum). You do not need to choose a Primary Care Dentist.
